I. Application for Notification Letter of Foreigner’s Work Permit for a Z visa (over 90 days)
Checklist ：
1. Passport information page;
2. Signed and Stamped Contract;
3. Passport-style photo in white background (jpg format, 40-120k);
4. Diploma of highest degree (shall be verified by Chinese Embassy, both original and Chinese translations are necessary);
5. Certificate of Non-Criminal Records (shall be verified by Chinese Embassy, both original and Chinese translation are necessary);
6. Physical Examination Report done within 6 months (shall be issued by foreign or Chinese medical organizations verified by the Chinese Inspection and Quarantine Bureau);
7. Filled information form;
8. Certificate of previous working experience (both original and Chinese translation is necessary)
9. Documents of accompanying family members (relationship certificate shall be verified by Chinese Embassy).
II. Police registration within 24 hours after Entry

III. Verification of Physical Examination Report
Make appointment with Jiangsu International Travel Health Center on WeChat (江苏国际旅行卫生保健中心) and go to the recognized hospital at the supposed time, address: No. 39, Chuangzhi Road, Nanjing, Tel:025-52345700. If your report from abroad meets the requirements, it will be changed to a new report; If not, you need to do a checkup again. It will take 4 working days to get the new report.
IV. Checklist for Work Permit Application
1. Application form；
2. Passport information page, visa page and entry stamped page;
3. Employment Contract；
4. Valid Physical Examination Report;
5. All originals of documents used in the process before entry need to be checked.
V. Checklist of Residence Permit Application
1. Letter from NXU (Prepared by International Exchange Office);
2. Work Permit information page stamped by NXU;
3. Application form;
4. Passport original and copies of information page, visa page, and entry stamped page;
5. Work permit card (original and copy);
6. Passport-style photo with white background (within 6 months);
7. Accommodation registration form from local police station;
8. Valid Physical Examination Report;
9. Accompanying family members’ document (if available).
